Clinically appropriate nutrition support, without unnecessary friction

Eat Well Health works with dietitians to make targeted nutrition support clinically appropriate, administratively simple, and accessible at scale for older adults living at home under Support at Home and Home Care Packages.

We start from a shared position with dietitians: food comes first.

However, you will also be familiar with the reality of practice — many older adults are unable to meet nutritional requirements through food alone due to reduced appetite, fatigue, illness, functional decline, or social factors. In these cases, timely nutrition support is often required to prevent further deterioration and support independence. Eat Well Health has been designed to support this reality, while avoiding the common barriers that prevent good recommendations from being followed through consistently.

The problem we are solving

Dietitians regularly identify clients who would benefit from nutrition support, yet:

  • Access is inconsistent or delayed
  • Providers are unsure how to claim or justify support
  • Clients face administrative friction
  • Recommendations are not actioned, or lapse over time

The result is a gap between good clinical advice and real-world implementation.
Eat Well Health exists to close that gap.

A dietitian-led model

Eat Well Health is not a supplement company operating independently of clinical care.

Our model is explicitly dietitian-led:

  • Nutrition support is provided only where clinically justified
  • A dietitian recommendation underpins access and claiming
  • The recommendation remains central to the client record
  • Products are positioned as supportive to food intake, not replacements by default

This ensures nutrition support is:

Clinically appropriate

Fundable

Defensible

Sustainable over time

Nutrition screening and clinical oversight

Eat Well Health incorporates a short, structured nutrition screening tool designed for remote use with older adults living at home.

The screening:

  • Is based on the principles of validated nutrition risk screening tools used in older populations including the MNA-SF.
  • Identifies whether a client is:
  • At risk of malnutrition
  • Experiencing suboptimal intake
  • Likely meeting requirements

Based on the screening outcome, a dietitian-approved recommendation framework supports the generation of an appropriate recommendation or allows you to upload your own.

Importantly:

  • The system does not replace dietitian judgement
  • It provides a consistent, scalable way to support it
  • Dietitians retain full discretion over final recommendations

This approach balances clinical integrity with the realities of scale under Support at Home

How the clinician portal supports your practice

The Eat Well Health clinician portal has been designed to reflect how dietitians actually work.

Through the portal you can:

  • Register a client in minutes
  • Upload an existing dietitian recommendation or generate one aligned to funding requirements
  • Select the specific product(s) you are recommending
  • Provide clear clinical justification in plain language
  • Update or reuse recommendations as the client’s needs change

Once submitted:

  • The recommendation is stored with the client record
  • It can be referenced by providers to support claiming
  • It reduces the need for repeat explanations or paperwork

Outcome

Greater confidence that funding will be approved, and greater likelihood that the client will remain consistent with nutrition support.Greater confidence that funding will be approved, and greater likelihood that the client will remain consistent with nutrition support.

Alignment with Support at Home
and Home Care Packages

Eat Well Health products are:

  • Classified as Food for Special Medical Purposes (FSMP)
  • Designed specifically for the nutritional needs of older adults
  • Commonly claimed under Clinical Support – Nutrition & Dietetics, when supported by a dietitian recommendation

Eat Well Health does not replace provider care planning processes. Instead, we support them by:

  • Translating dietitian recommendations into a form providers can confidently act on
  • Aligning documentation with claiming requirements
  • Managing invoicing and logistics

This reduces uncertainty for providers and removes access barriers for clients.
