At Eat Well Health, we are passionate about improving the nutrition levels of senior Australians.
One of the barriers to improving the nutritional status of seniors is access to quality dietary advice and nutritional supplements. Under the Government Support At Home Package scheme, nutritional advice and certain supplements may be supported, subject to your provider's assessment and approval.
We have worked with our dietitians to develop an online nutrition platform. This platform includes the nutrition assessment screening tool and product recommendation algorithms, to provide you with tailored nutrition recommendations. It is then seamlessly sent to your Support At Home Provider enabling you to order the recommended products.
Once your Support At Home Provider approves the recommendation, the products can be dispatched directly to you and may be funded through your Support At Home Plan.
To determine your eligibility and obtain approval for nutrition support products, please consult with your Support At Home Provider. They can assess your individual needs and confirm whether these products can be included in your plan.
Our Simple 3-Step Process for Support at Home Clients
- Nutrition Screening: Complete a short online questionnaire. Our dietitian reviews your answers and confirms eligibility for funded nutrition support.
- Provider Coordination: We contact your Support at Home provider to manage documentation and claiming under Clinical Support – Nutrition & Dietetics.
- Delivery & Follow-Up: Your shakes are delivered monthly. We keep your provider’s records updated and check in to ensure the products continue meeting your needs.
