If you’re already approved for Home Care, your eligibility rolls into Support at Home. New clients can complete our quick nutrition screening to receive a dietitian recommendation that supports their application.
We work directly with providers across Australia to set up claiming and supply. We can liaise with your provider on your behalf once you submit your screening form.
If you have a Support At Home Plan and have been recommended a formulated meal replacement or protein supplement by a qualified health professional, you may be eligible for subsidy, subject to your provider's assessment and approval. You need to discuss this with your Support At Home Provider. Optimal Essentials Complete Nutrition - Pharam is a food for special medical purposes and is a formulated meal replacement. Protein Powder - Pharama for Seniors is also a food for Special medical purposes complies as it is a formulated supplementary food. Both comply with the FSANZ Standard 2.9.3 which is a requirment of the Support At Home Plan guidelines.
Other Optimal Essentials products are not considered formulated supplementary foods under the FSANZ standard and therefore are not available for subsidy through a Support AT Home Plan.

If you order Complete Nutrition or Protein Powder for Seniors through your Support At Home System, we will invoice your Support At Home Provider direct and once they approve your order we will have the product delivered directly to your home. If your Support At Home Provider approves the recommended products, we can invoice them directly, potentially reducing or eliminating your out-of-pocket expenses.
To determine your eligibility and obtain approval for nutrition support products, please consult with your Support At Home Provider. They can assess your individual needs and confirm whether these products can be included in your plan.
If you decide to order additonal products which have been recommended for you they are not eligible under the Support At Home Package and will have to be paid at the time of order.
Upon completion of the Questionnaire, the recommendation from our dietitian will immediately come up on your screen, and an email will be sent to you and your Support At Home Provider.
To review your Questionnaire results, orders, and communication with us, you need to login into the system by clicking on the Dashboard menu item. You will then need to put in your login email address and password.

Yes the Nutrition Assesement Questionnaire is available for everyone over the age of 65. It has been specifically been designed for Seniors. You do not need to be on a Support At Home Package.
Yes you can setup a login for a family member or carer when you first complete the Questionnaire which will allow them to receive copies of your nutrition assesment and any orders you make. They can even make orders on your behalf if you would like.
You are able to order your products on a subscription so you make sure you never forget to order your products and are left short. The subscription can be cancelled or modified at any time.
You can send messages to our team of in house dietitians or can book an online clinical appointment for a detailed nutrition assesment through our online portal with one of our supporting dietitians.
