Do you already have a written recommendation from your doctor or dietitian?

If you already have a written recommendation for a formulated meal replacement or protein powder there is no need to complete the nutrition screening and questionnaire. You can order the products direct via the Support At Home Provider system. You can either upload a copy of the recommendation from your computer or send a copy directly to your Support At Home Provider.

If your doctor or dietitian has recommended a nutrition support product or more protein, you do not need to complete our questionnaire. Just upload your written recommendation from your doctor or dietitian and we will send it with your order to your Support At Home Provider.

Order using your own recommendation

If you already have a recommendation from your own dietitian, upload it when you set up your account. We supply the products your dietitian has recommended, and your Support at Home provider approves the order.

To help you improve your nutrition we have developed a system to make it easy to order Complete Nutrition – Pharma and Protein Plus for Seniors – Pharma. Both products are designed to meet the nutritional needs outlined in the government's Support At Home guidelines. However, inclusion in your plan is subject to assessment and approval by your Support At Home Provider.

If you have already received a recommendation from your doctor or dietitian there is no need to go through our nutrition screening tool to order products through your Support At Home Package from your Support At Home Provider.

This may enable you to have the products covered under your Support At Home Plan, subject to your provider's assessment and approval.

To determine your eligibility and obtain approval for nutrition support products, please consult with your Support At Home Provider. They can assess your individual needs and confirm whether these products can be included in your plan.
