You can order the products direct via the Home Care Provider system. You can either upload a copy of the recommendation from your computer or send a copy directly to your Home Care Provider.

If your doctor or Nutritionist has recommended you take a formulated meal replacement or increase the amount of protein as part of your nutritional intake then there is no need to take our assesment. Just upload your written recommendation from your doctor or dietitian and we will send it with your order to your Home Care Provider.

To help you improve your nutrition we have developed a system to enable you to easily gain access to Optimal Essnetials Complete Nutrition Pharam our formulated meal replacement and Protein Powder for Seniors Pharma which is a formulated supplementary food. Both products are designed to meet the nutritional needs outlined in the government's Home Care Package guidelines. However, inclusion in your plan is subject to assessment and approval by your Home Care Provider.

If you have already received a recommendation from your doctor or dietitian there is no need to go through our nutrition screening tool to order products through your Home Care Package from your Home Care Provider.

This may enable you to have the products covered under your Home Care Plan, subject to your provider's assessment and approval.

To determine your eligibility and obtain approval for nutrition support products, please consult with your Home Care Provider. They can assess your individual needs and confirm whether these products can be included in your plan.
