Complete a short, guided screening to assess your nutrition status, confirm your eligibility for funding, and allow our dietitians to determine whether you will benefit from the products.

Step 2 — Results & Order Options

Based on your screening results, you’ll receive a personalised assessment of your nutrition status, product recommendations from our dietitians, and the option to order approved products funded through your Support At Home Package. You can begin with a one-off order or choose an ongoing subscription for convenience.

Step 3 — HomeMade Approval

Your HomeMade coordinator reviews your order, ensures it fits within your funding package, and approves it for supply.

Step 4 — Delivery & Ongoing Support

Once approved, your products are despatched directly to your home. Our team provides ongoing support and simple options to re-order or adjust your plan. Choosing an ongoing subscription is the easiest way to ensure you always have the products you need, without the hassle of re-ordering. You can pause or cancel at any time.

Set Up an Account

To place your first order, you’ll need to set up an account with Eat Well Health. This ensures your details are linked correctly with your HomeMade package.

  • Email address
  • Full name
  • Home address
  • Your My Aged Care number or Homemade Support number

After Your Screening

After completing the screening questionnaire you will receive:

  • A personalised assessment of your nutrition status
  • Product recommendations as determined by our dietitians
  • The option to order the recommended products funded through your Support At Home Package
  • Regular communication on the status of your order

Maggie Beer’s aged care meal reform has reignited debate about food quality in residential care—but the real gap may be at home, where most older Australians live. Despite over a million people receiving government-funded care at home, nutrition initiatives still focus almost entirely on facilities. At Eat Well Health, we’re tackling this challenge differently.
