How Eat Well Health works in practice

Eat Well Health provides a simple, dietitian-supported pathway that helps Trilogy Care clients access clinically appropriate nutrition support shakes when meals alone are not enough.

The model is designed to identify nutrition risk earlier and enable practical support — without adding workload for Trilogy staff or requiring changes to existing processes.

Step 1: Identifying a client who may benefit

Clients may be identified through:

  • routine care and monitoring
  • onboarding or care plan reviews
  • concerns about appetite, weight loss, frailty or reduced strength

No clinical assessment or decision-making is required by Trilogy staff.

If there is any concern, the simplest next step is to direct the client to the Eat Well Health Trilogy page to complete the screening.

Step 2: THE CLIENT COMPLETES A SHORT ONLINE NUTRITION SCREEN

In most cases, the client simply completes a short online nutrition screening:

Simple online screening (2–3 minutes)

The client (or their carer/family member) completes a short online nutrition questionnaire.

Based on their responses, the screening generates a structured nutrition classification and recommendation. Where required, this can be supported by dietitian input.

No involvement is required from Trilogy staff beyond sharing the link.

If the client already has a GP or dietitian recommendation

If the client already has a GP or dietitian recommendation, it can be uploaded instead.

No additional screening is required.

  • Most clients complete the screening themselves or with help from family
  • The simplest next step is to send the client the Trilogy Care page
  • This allows them to complete the screening and access support without delays

Step 3: Dietitian support (Managed for You)

Dietitian involvement is managed within the Eat Well Health model, so clients receive appropriate clinical support without adding workload for Trilogy staff.

Additional support where required

Clients complete a short, remote-friendly nutrition screen to identify who may benefit from additional support. Screening can be used at onboarding, routine reviews, or when appetite, intake, weight or strength are changing.

Escalation to selectively

When screening indicates risk, clients can be escalated for one-on-one dietitian support in the model that suits you:

  • Your in-house dietitians: full access to our portal to view screening results and upload or complete recommendations.
  • Eat Well Health telehealth dietitians: ideal if you don’t have dietitians on staff, and also useful for surge capacity when internal teams are stretched.

Works alongside existing clinical care teams

Eat Well Health is designed to work alongside your current clinical relationships. In-house or external dietitians can log in and recommend clients directly, or upload their own recommendations to support access to nutrition support shakes.

We handle the rest

Once a recommendation is in place, we handle the practical steps — documentation alignment, provider liaison, invoicing and delivery — to help ensure support is implemented consistently.

Step 4: NO ADDITIONAL WORK FOR TRILOGY STAFF

Eat Well Health is designed to fit within Trilogy Care’s existing processes, without requiring staff to manage new systems or workflows.

Depending on your model, this may include:

  • A standard invoice with supporting documentation for processing
  • No requirement for staff to complete assessments or manage clinical decisions
  • No need to log into a portal (unless desired)

In most cases, Trilogy staff are not required to take any action beyond directing the client to the Eat Well Health page.

Step 5: SIMPLE ORDERING, DELIVERY & CLAIMING

  • Clients order recommended products online
  • Products are delivered directly to the client’s home
  • Eat Well Health invoices Trilogy Care directly
  • Supporting dietitian documentation is included with each invoice
  • For eligible clients, this is funded under Support at Home (no co-contribution required)

Once a client is identified as suitable, Eat Well Health provides dietitian-designed nutrition support shakes for older Australians.

Use your in-house dietitians

If Trilogy Care has dietitians on staff, they can remain fully in control of clinical decisions.

  • your team can use Eat Well Health to support online nutrition screening and ongoing monitoring
  • provider dietitians can upload recommendations / prescriptions to support funded nutrition supply
  • your team retains full clinical oversight, while Eat Well Health manages supply and documentation

This approach suits providers who already have dietitian capacity and want a simple way to implement nutrition support at scale.

OPTION 2:
Eat Well Health dietitian support via telehealth

Where internal dietitian capacity is limited, Eat Well Health can provide dietitian support as part of the pathway.

In this model:
