Step 1 — Nutrition Screening

Complete a short, guided screening to assess your nutrition status, confirm your eligibility for funding, and allow our dietitians to determine whether you will benefit from the products.

Step 2 — Results & Order Options

Based on your screening results, you’ll receive a personalised assessment of your nutrition status, product recommendations from our dietitians, and the option to order approved products funded through your Home Care Package. You can begin with a one-off order or choose an ongoing subscription for convenience.

Step 3 — Trilogy Care Approval

Your Trilogy Care coordinator reviews your order, ensures it fits within your funding package, and approves it for supply.

Step 4 — Delivery & Ongoing Support

Once approved, your products are despatched directly to your home. Our team provides ongoing support and simple options to re-order or adjust your plan. Choosing an ongoing subscription is the easiest way to ensure you always have the products you need, without the hassle of re-ordering. You can pause or cancel at any time.

Set Up an Account

To place your first order, you’ll need to set up an account with Eat Well Health. This ensures your details are linked correctly with your Trilogy Care package.

Minimum information required:

• Email address
• Full name
• Home address
• Your My Aged Care number

• Tip: It will speed up your registration if you also have the name and email of your Trilogy Care team member who oversees your account. This is not essential but can help us link your details more quickly.
