What we need to secure Support at Home Funding
To place your first order, you’ll need to set up an account with Eat Well Health. This ensures your details are linked correctly with your Support at Home package.

- Your Support at Home Provider Details
- Tip: It will speed up your registration if you also have the name and email of your Support at Home provider team member who oversees your account. This is not essential but can help us link your details more quickly.

After completing the screening questionnaire you will receive:
