Eat Well Health’s online nutrition screening provides Support at Home providers with a simple, standardised way to assess nutrition risk as part of client onboarding, care planning and ongoing review.

The screening supports timely identification of nutrition concerns and provides dietitian-reviewed documentation to help providers support appropriate intervention — including access to clinically formulated nutrition support shakes where required.

Why nutrition screening matters for providers

Nutrition plays a critical role in maintaining strength, function and independence in older Australians. However, nutrition risk is often under-identified or documented inconsistently, particularly at the time of onboarding.

Eat Well Health’s screening helps providers by:

How the nutrition screening works

The screening process is designed to be quick and low-burden:

  • Clients complete a short online nutrition screening

  • Responses are reviewed by a dietitian

  • A recommendation is prepared where clinically appropriate

  • Documentation is available to support care planning and funding

Where a client already has a GP or dietitian recommendation, screening is not required. Existing documentation can be used instead, avoiding duplication and delays.

Supporting access to nutrition support shakes

Where screening identifies nutrition risk and additional support is clinically appropriate, Eat Well Health supports access to clinically formulated nutrition support shakes designed for older Australians.

These shakes are commonly used when:

  • appetite is reduced

  • meals are missed or intake is inconsistent

  • weight loss, frailty or reduced strength are concerns

Eat Well Health coordinates dietitian review, documentation and supply, making it easier for providers to support this intervention without adding operational complexity.

Ongoing monitoring over time

Nutrition needs change as a client’s circumstances change.

Eat Well Health supports repeat nutrition screening where appropriate, allowing providers to:

  • document changes in nutrition risk over time

  • respond early to declining intake or weight loss

  • demonstrate ongoing monitoring as part of care delivery

This supports a more proactive, preventative approach to nutrition under Support at Home.

Designed to fit different provider models

Nutrition screening can be used across different provider operating models, including fully managed and self-managed approaches.

Not all provider staff require portal access. Screening and documentation can be supported in a way that aligns with each organisation’s governance and workflow preferences.
