A simple way to support funded nutrition for your clients
The Eat Well Health portal provides dietitians with a simple, secure way to support access to funded nutrition supportfor eligible clients living at home.
It is designed to fit easily into clinical workflows and to minimise administrative effort, while ensuring documentation is available to support 100% subsidy under Support at Home where eligibility criteria are met.
What dietitians use the portal for
Dietitians typically use the portal to:

upload a nutrition support recommendation for a client

register a client during or after a consultation
-
support access to nutrition support with full subsidy for eligible clients
-
view client history where relevant
The portal is not intended to replace clinical records or practice management systems.
Uploading a recommendation
When you determine nutrition support shakes are clinically appropriate:

your recommendation can be uploaded directly to the portal
-
you may upload your usual clinical recommendation, or use an Eat Well Health recommendation template where helpful
-
this documentation is used to support eligibility and funding alignment
-
no additional referrals or approvals are required from you
Use of the Eat Well Health template is optional and intended to save time. Clinical judgement and wording remain yours.
For eligible clients, uploading a recommendation enables access to nutrition support shakes with 100% subsidy through their Support at Home plan.
Registering a client during a consultation
Some dietitians choose to:

register a client in the portal during the consultation, or
-
complete registration shortly afterwards
This allows nutrition support to be implemented promptly and reduces delays that can occur when clients are required to coordinate access themselves.
What happens after you upload
Once a recommendation is uploaded:

Eat Well Health manages funding alignment and documentation
-
nutrition support shakes are supplied directly to the client
-
providers receive supporting documentation where required
-
ongoing supply is managed where clinically appropriate
Dietitians are not required to manage supply, delivery or invoicing.
