At the centre of the Eat Well Health model is a small range of ready-to-use nutrition support shakes designed by dietitians, for the nutritional needs of older adults living at home.
These products are nutrition shakes intended to complement — not replace — food-based strategies where intake is insufficient, appetite is reduced, or nutritional requirements are difficult to meet through diet alone.
Dietitians decide if and when these products are appropriate, based on individual client need and clinical judgement.
For eligible clients, these products can be accessed with 100% subsidy through the client’s Support at Home plan when supplied as part of a documented, dietitian-led nutrition service.
Clinical context
A food-first approach remains the foundation of nutrition care. However, in older adults living at home, dietitians frequently see situations where food alone is not sufficient.
Common contributing factors include:

fatigue, frailty or functional decline

difficulty preparing or consuming adequate meals

increased energy or protein requirements
In these situations, clinically appropriate oral nutrition support can play an important role in meeting nutritional requirements and supporting strength, function and independence.
Where clients are eligible under Support at Home, Eat Well Health removes cost as a barrier by enabling access to nutrition support with full subsidy.

Eat Well Health supplies a small, purpose-designed range of pharma-grade nutrition support products for older adults. Product selection is guided by clinical assessment and intended use.
Both products are eligible for 100% subsidy for eligible Support at Home clients when supplied as part of a dietitian-led clinical service.
Complete Nutrition – Pharma
Designed for clients who require broad nutritional support where overall intake is inadequate.

Complete Nutrition is commonly used where:

Designed for older adults with higher protein requirements, particularly where maintaining muscle mass and functional capacity is a priority.

Clinical use and monitoring
Eat Well Health nutrition support products:

Clinical decisions regarding product selection, duration of use and monitoring remain with the treating dietitian or GP.
Funding and eligibility
Eat Well Health nutrition support products are formulated to meet nutrition eligibility and audit expectations under the Support at Home guidelines when supplied as part of a documented, dietitian-led clinical service.
Where clients meet eligibility criteria, products are supplied with 100% subsidy through the client’s Support at Home plan, with no out-of-pocket cost to the client.
