How It Works
Your role in supporting funded nutrition for Support at Home clients
Eat Well Health supports dietitians to help eligible clients living at home access clinically appropriate nutrition support shakes with 100% subsidy through their Support at Home plan, where this forms part of a documented nutrition intervention.
This page outlines what is required from you as a dietitian when working with a Support at Home client.
Step 1: Assess your client
You see a client living at home who is receiving Support at Home services.
Based on your clinical assessment, you determine that nutrition support shakes are clinically appropriate — for example due to:

reduced appetite
-
poor oral intake
-
weight loss, frailty or reduced strength
-
difficulty meeting nutritional needs through meals alone
Clinical judgement remains entirely with you.
Step 2: Prepare a recommendation
You prepare a nutrition support recommendation for your client.
This may be:
-
your usual clinical recommendation, or
-
a recommendation prepared using an Eat Well Health template (if preferred)
The recommendation supports access to nutrition support under Support at Home.
For eligible clients, this recommendation enables access to nutrition support shakes with 100% subsidy through their Support at Home plan.
Step 3: Upload the recommendation
The recommendation is uploaded to the Eat Well Health portal for the client.
This can be done:

during the consultation, or
-
shortly after
The uploaded recommendation is used to:

support eligibility for funded nutrition support
-
provide documentation for the client and their provider
-
enable supply of nutrition support without additional referrals
No additional approvals or referrals are required from you.
Step 4: Eat Well Health coordinates access and supply
Once the recommendation is uploaded:
-
Eat Well Health coordinates access to clinically formulated nutrition support shakes designed for older Australians
-
documentation is provided to support funding and provider requirements
-
nutrition support is delivered directly to the client
-
ongoing supply is managed where clinically appropriate
For eligible clients, nutrition support is supplied with 100% subsidy through their Support at Home plan, with no out-of-pocket cost.
What dietitians are not required to do
Eat Well Health is designed to support implementation, not add administrative burden.
Dietitians are not required to:

source or manage nutrition products
-
coordinate funding or invoicing
-
liaise with providers about payments
-
provide ongoing administrative follow-up
Your role remains focused on clinical care.
