Funded access to nutrition support under Support at Home

Where eligibility criteria are met, Eat Well Health enables older clients living at home to access clinically appropriate nutrition support shakes with 100% subsidy through their Support at Home plan, as part of a documented, dietitian-led nutrition intervention.

This removes a major barrier to long-term adherence and allows clinically appropriate nutrition support to be implemented and maintained.

What funding applies

Nutrition support supplied through Eat Well Health is accessed under Clinical Supports – Nutrition & Dietetics within the Support at Home program, where eligibility criteria are met.

This funding supports:

  • dietitian-led nutrition assessment and intervention

  • provision of clinically formulated nutrition support shakes

  • documentation to support care planning and compliance

For eligible clients, nutrition support is fully subsidised, with no out-of-pocket cost.

Eligibility considerations

Eligibility is determined based on:

  • the client’s Support at Home status

  • clinical need as identified by a dietitian or GP

  • documented nutrition intervention

Dietitians do not need to determine funding eligibility themselves.

Eat Well Health supports the funding alignment and documentation process.

Documentation requirements

To support funded access, Eat Well Health requires:

  • a dietitian or GP recommendation for nutrition support

  • documentation outlining clinical rationale

  • records to support provider and audit requirements

Where a dietitian provides a recommendation, this documentation is used to:

  • support eligibility for funding

  • enable access to nutrition support with full subsidy

  • provide assurance to providers and auditors
