IMPORTANT: Orders made for plan managed accounts will be sent to your provider and once approved by your provider will be despatched to you and invoiced direct to your provider. You will be able to follow the progress of your order by logging into your client account on the eatwellhealth.com.au website. Once you have completed and submitted the form below you will be emailed your account details allowing you to access your account and place orders.

Please note that fields marked with an asterisk (*) are required fields and must be filled in to proceed.


If you provider is not listed in the drop-down menu, please select the “My Plan Manager is not listed” option and you will asked to provide your provider contact details.
